Please be prepared to answer general questions such as county residence, date of birth, gender, martial status, insurance and income. In addition, you will be asked the reason for your call and what you are currently experiencing. This will help us to determine how we can best serve you.
Your call, as well as your information, is strictly confidential. Dream Provider Care Services and all its contracted providers are subject to the "Health Insurance Portability and Accountability Act of 1996" (HIPPAA). This federal law protects the privacy and confidentiality of anyone receiving mental health, developmental disability, substance abuse, or any other services being provided.
